East Integrated Care Community Team (ICC)
We are a team of NHS health care professionals based in local surgeries covering a large geographical area including Carnforth, Arnside, Milthorpe, Kirkby Lonsdale, Sedbergh and Bentham - and areas in between. Our aim is to maximise individuals’ full potential, independence and well-being and help to make sure your wishes are heard. We access many sources of support and information from within the health sector, the voluntary sector and social care, as well as wider community assets.
This can include signposting as appropriate to local groups and statutory agencies.
Our goal: To promote independence, awareness of health and well-being , reduce the risk of hospitalisation, and support people when they are discharged from hospital.
All GP registered patients aged 18 years or older from the following surgeries are eligible for support from our East ICC Case Management Team - Park View Surgery, Arnside Surgery, Stoneleigh, Lunesdale Surgery, Sedbergh Medical Practice, and Bentham Medical Practice.
Referrals can be made by anyone, as long as they have obtained consented from the person who is being referred (this may include your GP, other health & social care professionals, or you can self-refer).
A case manager/care coordinator/care navigator, dependent upon need, will be assigned as your point of contact.

· To enhance your well-being and encourage a good quality of life
· To help you understand any long term conditions you may have and manage these well
· To avoid any inappropriate admissions to hospital
· To help you live as independently as possible
· To enhance the care that you receive in your own home
· To help you understand your medications
· To discuss and document, at your request, the things that are important to you about your future
We work in collaboration with a variety of organisations (groups, charities and other 3rd sector groups) to ensure we are providing the best joined-up care for people living in our local communities.
