Craven Communities Together Partnership is one of thirteen community partnerships within Bradford District and Craven Health and Care Partnership. Our vision is that people will be "Happy, Healthy at Home" with improved population health through integrated health, care and support. We have commitment to shared values and behaviours which support effective collaborative partnership working. Membership includes representation from a wide and diverse range of organisations and groups supporting the Craven population.

Craven Communities Together Partnership members meet by invitation on a monthly basis to celebrate innovation and good practice; share information and identify priorities; provide networking and information sharing opportunities; and promote new ways of working together.
